Residential Care Admissions

The Omaha Home for Boys Residential Program is generally for boys and young men ages 10-18. The Parent or Legal Guardian must complete an application form. Download a Residential Care application form; or call, (402) 457-7000.

Requirements

A Psychological Evaluation from the past year, including an IQ score, is needed to start the application process. Additional documentation may be requested at this time. After completion of the application, the Admissions Coordinator will schedule an interview and campus tour with the youth and legal guardian. If a youth is accepted into the program, additional documentation is required prior to intake. This will be followed by developing a treatment/reunification program for the youth and guardians.

Jacobs’ Place Admissions

Jacobs’ Place is a transitional living program to assist young men and women, generally ages 17-21, who are “aged out” or about to “age out” of foster care. An applicant must complete the Jacobs’ Place application. Download a Jacobs’ Place Admission Form.

Requirements

A Psychological Evaluation or Mental Status Exam from the past year must be submitted. A grade audit is also required for applicants who do not have their high school diploma or GED. After completion of the application, the Admissions Coordinator schedules an interview with the applicant. If the applicant is accepted into the program, additional documentation may be required prior to intake. An intake meeting and tour is scheduled with the applicant (and legal guardian for applicants under the age of 19). Applications must be filled out and mailed, emailed or faxed to:

Branching Out Admissions

Branching Out  is an independent living program for young women and men ages 14 to 24. Many of these youth are in foster care or are former foster care youth. Download a Branching Out Application Form. Applications must filled out and mailed, faxed or emailed to:
